The Transitional Care & Rehabilitation Unit at Logan Regional Hospital provides individually structured rehab services to patients who can benefit from daily physical, occupational or speech therapies or skilling nursing care.
Transitional Care & Physical Rehabilitation is a separately licensed part of the hospital called a skilled nursing facility and is certified by Medicare. It is designed for a short-term stay to increase the resident’s strength and wellness to enable a better transition to home or another living center. Skilled care means health care services given when you need skilled nursing or rehabilitation staff. You skilled needs will be reassessed daily. Once your goals have been met and/or you reach your potential, your care team will work with you to develop a plan for your safe discharge.
You will have access to a physical therapist when needed and 24-hour registered nursing care while you are in the unit. You and your family will also be involved with an interdisciplinary team designed for your specific care plan. This team may include a physician; a physical, occupational or speech therapist; a social worker; a dietician; a pharmacist and a recreational therapist.
