Home Care

Many people feel most comfortable in their own home. This doesn’t grow less true with age. If your elderly loved one requires care and assistance but are loathe to leave their home, it is no problem! We offer effective home care services to provide superior care for your loved ones, without the inconvenience of leaving their homes.

Home health care is a wide range of health care services that can be given in your home. Home health care is less expensive and more convenient than the care you get in a hospital or skilled nursing facility. The goal of home health care is to treat an illness or injury. Home health care helps you get better, regain your independence, and become as self-sufficient as possible. Services may also include medical social services or assistance from a home health aide. The home health care agency coordinates the services your doctor orders for you.

Hospice Care

For this reason, we provide hospice care wherever the individual lives, whether it is in a nursing home or their own house.

Hospice is a wonderful benefit provided for those facing a life-limiting illness. Hospice care offers emotional and physical care to allow for the best quality of life. Too often patients and families wait until the last days or weeks of life to elect hospice when they could have enjoyed months of very helpful hospice services. The hospice team consists of a physician, nurse, social worker, spiritual support, and aides where needed.

Hospice services may include:

Private duty services are helpful to nurture independence, which can be particularly valuable for individuals that fear being dependent on others. It typically involves helping with personal care, as well as light cleaning in the house. Private duty care is offered wherever the individual resides.

Applegate is pleased to offer private duty wherever you call home. Private duty is a personal care service that fosters independence and comfort. Under a nurse’s supervision, a Certified Nursing Assistant (C.N.A.) helps the individual with personal cares and/or light housekeeping.
